Cosmetic Non-Anesthetic Pet Teeth Cleaning – Consent & Liability Waiver

Pet Owner Declaration

I, the undersigned pet owner, confirm and agree that:

1. My pet is in good general health, with no known pre-existing conditions that would make cosmetic teeth cleaning unsafe, and is current on vaccinations required by local or state law, if applicable.

2. My pet is friendly, calm, and able to safely tolerate handling required for the service.

3. I understand that Bright N’ Shine Pet Teeth Cleaning® provides a cosmetic, non-veterinary, non-medical, and non-anesthetic grooming service, focused solely on improving oral appearance and hygiene.

4. I understand that no anesthesia, medications, or chemical agents are used, and that this service does not replace veterinary dental care. Any diagnosis, treatment, or procedures below the gumline must be performed only by a licensed veterinarian. The service is limited to cosmetic removal of surface plaque and tartar and does not invade areas restricted to veterinary practice.

5. I accept full responsibility for my pet’s health, condition, and behavior before, during, and after the service, and I release Bright N’ Shine Pet Teeth Cleaning® from any liability arising from pre-existing conditions, behavioral reactions, or unforeseen events beyond the company’s control.

Note: Avoid food for at least 30 minutes prior to the appointment to reduce the risk of discomfort or regurgitation.

Loose or Falling Teeth Disclaimer

I understand that loose, fragile, or damaged teeth may naturally detach during or after cleaning. Such teeth are often already compromised and held in place only by tartar buildup. The cleaning process may reveal pre-existing weakness and does not cause dental loss.

Aggressive Pet / Owner Responsibility Clause

I confirm that my pet has no history of aggression, biting, or attacking and can safely interact with strangers. If my pet displays aggressive, fearful, or uncontrollable behavior, the technician may immediately refuse or stop the service for safety reasons.

Senior Pet / Special Conditions Waiver

I acknowledge that senior pets or pets with pre-existing conditions (including loose teeth, fragile gums, heart or respiratory issues) may experience unforeseen reactions or emergencies. Despite all reasonable precautions, I agree that Bright N’ Shine Pet Teeth Cleaning® cannot be held liable for injury, reaction, or sudden death resulting from such conditions or events beyond its control (Acts of God).

Liability Waiver & Hold Harmless Agreement

While we take all reasonable and necessary precautions to ensure the safety and comfort of your pet, Bright N’ Shine Pet Dental shall not be held liable for any injuries, medical conditions, or health issues that are not caused by, related to, or the result of our services, including but not limited to pre-existing conditions, underlying health issues, or unforeseen reactions that may arise during or after the service.
